If you have never filled out an application with us before and would like to do so, you may download the application and send it back to us as an attachment. Please read the following instructions first:

1) Click on the link for the Participant Application. When prompted, click Save, and then title it with the last four digits of your Social Security Number. Make sure to save it where you can find it to reopen it.
2) Reopen the Application, Fill it out, and save it again to save your changes. Make sure you are thorough. Dates are formatted as MM/DD/YYYY. Phone numbers are formatted as (000) 000-0000, type only numbers and it will be formatted for you. Do not use Enter to switch from field to field. Press Tab, use the arrow keys, or select each field with your mouse.
3) Email the application back to us at recruiting@bkcaus.com as an attachment. If you have any questions, email us or call 831-2048.
